The Process

Clients, please complete the steps below prior to admission.

Rehabilitation Clients

Prefer to print out the forms to complete? CLICK HERE to download a fillable PDF that includes the Intake Form, Liability Release, Walker Release, and Horse Advance Directive.

Step One:

Complete the following forms:

Click on each form below, then enter the password sent to you by OHF to access and submit.​

Step Two:

Submit the following documentation:

​

  1. Recent (last 60 days) vet assessment/prognosis and rehab plan (If unavailable, an intake meeting is required prior to admission)

  2. Horse Advance Directive

  3. Insurance Information

Step Three:

Once the forms and documents are submitted, you will:

​

  1. Receive a treatment timeline and financial estimate from OHF via email

  2. Approve the timeline and estimate

  3. Sign a financial agreement

  4. Schedule and confirm your admission day with OHF via email

Step Two:

Once the forms and documents are submitted, you will:

​

  1. Receive confirmation of your retirement board choices and monthly estimate via email

  2. To finalize, review and reply to the email with your acceptance of choices and estimate

  3. Schedule and confirm your retirement start date with OHF via email

We kindly ask for 30 days' written notice before removing a horse from retirement board. This helps us plan accordingly and continue providing excellent care for all horses on the property.​
